Social service: help and advice for what's to come
What's going to happen after I leave hospital? Will I be able to cope at home alone? Or will I need help from a home nursing service? The social service in the Evangelische Geriatriezentrum Berlin (EGZB) deals with these and many more issues. Here you will find help and support relating to social aspects and other issues involved in what's to come when you leave the hospital. The services offered include for example:
- advice regarding rehabilitation and care insurance
- assessing your possible claims for state assistance
- preparing you for when you are discharged from the hospital
- triggering urgent assessment by the MDK (Medical Service of the Health Insurance Funds) if you have not yet been classified for attendance allowance.
Special attention is paid to discharge management. For example, if you decide to use the assistance provided by a home nursing service, we can work together with a corresponding firm of your choice to ensure that all important preparations are in place. This ensures that you will receive all the necessary care and attention as soon as you get home.
Furthermore, we offer you the possibility of talking to us about all your cares and needs and going through your particular circumstances. You can also approach us about the various issues involved in durable powers of attorney for health care, appointing legal representatives for health care and living wills.
